As a home health aide, your job is to provide hands-on patient care within a patient’s home, coordinate the services of other physicians or health care providers, communicate with health insurance providers, and supervise other nurses who visit the home. Your responsibilities also include developing a Plan of Care (POC) for patients based on their needs and feedback from their families, helping support adherence to the treatment plan, monitoring the patient’s needs, including wellness, medical, and specialist treatment, and recommending any community services required. Your additional duties as a home health care case manager include coordinating the implementation of the plan with the primary doctor, making changes as needed, and completing all required documentation.
Qualifications necessary to become a home health care case manager include a bachelor’s degree in nursing. If you have an associate degree, you can still work in this career, but you must either be a licensed practicing nurse (LPN) with two years of professional experience or a registered nurse (RN). You should have community connections to provide service referrals when needed, be aware of community resources, have a high level of cultural competency, and be involved in community outreach. It is vital that you are familiar with health information technology and how to complete the required documentation, including POCs, Medicaid CANS, and progress notes. You should be able to work well with a team and directly with patients and their families. Excellent communication skills are essential as well. When implementing Medicaid Managed Care, you must have the ability to review and collaborate with other managed care organizations.
